End users opt in via Website Form by visiting https://www.legacylife.co/requestagent and completing a form that collects their phone number. Users enter First name, last name and email and Phone (optional).- They see clear text: "Submit your details to get a quote. Check the boxes below to consent to SMS communications."- Two separate, optional checkboxes appear (not required to submit the form): 1. Transactional/Non-Marketing Consent: "By checking this box, I consent to receive text messages from a licensed life insurance agent with Legacy Life about Life insurance quotes, appointment reminders, and general communication regarding life insurance as well as policy updates. Message frequency varies, message & data rates may apply. Text HELP for assistance, reply STOP to opt out." 2. Marketing/Promotional Consent: "By checking this box, I consent to receive marketing and promotional messages including special offers, discounts, new product updates among others from a licensed agent with Legacy Life at the phone number provided.
